The bottle, kept in your fridge door contains a form filled in by you with a pen, giving your medical information which paramedics need to treat you properly and safely. They know it is there, because of the green cross stickers on your fridge door and facing your front door. The second answer for those who get about - a MedicAlert bracelet costing £19.95 plus the first years membership of £20. It can be free to those on limited income. On the back of the bracelet is your personal number, your main medical conditions and an emergency telephone number.
