A new Admiral nurse at the East and North Herts NHS Trust is helping to provide additional support for patients with dementia and their families – thanks to funding and support from Dementia UK.
Speaking about her role, Hannah Gardner said: “Working with Dementia UK is an amazing opportunity. It will allow me to work closely with those who have dementia and their families – offering one-to-one support, expert guidance and practical solutions to challenges.
“I will also be working with the community Admiral nursing team at Carers in Hertfordshire, to ensure patients with dementia have a supported pathway from admission into hospital to when they’re discharged.
“If families would like extra support, they can call me on 07917 849122 or visit me in my office on ward 9B at Stevenage’s Lister Hospital.”
Charlotte Harris, for Dementia UK, said: “Admiral nurses allow families to face dementia with more confidence.”
